Modular Messaging Survey Form

We are rolling out Modular Messaging and wanted to know if there was form out there that we could send to our employees after they've used the system for 1 month... something like a survey, how to improve, customer satisfaction, etc...?

Anyone seen something like this for MM?

We're migrating from a legacy Octel environment. If I poll them today I would rec've greater than 80% dissatisfaction...

It's relatively solid, but not liked by many.
