Hey folks we have a 5 site SCN, using SIP trunks. We upgraded them this summer from 10 up to 11.1.0.2. Turns out when you call extension to extension, the cell phone will get the caller ID of the 3 digit extension, so some carriers block this, meaning the call never gets to the users cell.

I have the SIP tab Name set to the Ten digit number. If you call the DID of that phone its fine.

How do I get the system to send out a caller ID to twinned cell phones, where it shows something other than the 3 digit extension?

Yeah. Avaya fecked this up on 11.0 and 11.1.
When I raised a case I got this is working as intended. The users SIP info was populated and calls into the ICR will Forward exactly as you want, however internals will fail due to invalid SIP headers. You can change the settings and get it to work, however on an ICR call you'll not get the original caller ID.

Carriers/SIP Trunk Providers normally use the PAID header for Caller ID. So this is what needs to be populated. If you go under the trunk as setup as below you can get it working.
