Hi,
In Sweden for example you can have your fixed line extension(DID) in your mobile phone as a service together with the operator, even if you have a on-prem PBX.
Does anyone know if this is a service available in Germany, UK, Ireland or Poland?
 
I think you may need to expand on that.

Do you mean your mobile # provided by the carrier?, or an actual DID&Ext number on a SIP app that is on your mobile?

"together with the operator"
For me that means your dialing for assistance with your carrier.

I think i´ve seen that feature on Ericsson MD110/MX-ONE PBXs, that is carrier dependent. I know for a fact that it is/was available on Spain by Telefonica, also in LatAm by Movistar/Telefonica. UK was also a market for this feature but i don´t know which carrier, if any, offered it.
 
O2 in the UK used to have their Mobex service, a company I used to work for had it and it worked well. It's going back a few years and I don't know if it's still available. They still have a service description available on their website.

I think you can have pretty much any number ported to SIP and your provider can then be accessed through a SIP client on your cell phone.
I know I have a German phone number on my cell like that.
