Mixed up attachments

I'm not sure if this is an Exchange issue, or an outlook issue on one users machine.

I have a user who twice in the last few months, has had this issue. They attached 2-3 files in an e-mail, and sent them off. The recipient e-mails them back and says the two attachments are the same content, but the file names are the the originals.
Example, they sent:
Brochure.pdf (800Kb)
Applicationform.pdf (108Kb)

The recipient sees the two attachments with the proper filename, but both are actually the applicationform.pdf and both 108Kb.

The user had deleted the e-mail from her sent items already (and deleted items), so I recovered it, and can see the problem exists on the exchange servers copy of the message.

The user sent the same 2 files to me, and they came across normally, so this looks like an intermittent issue.

Anyone ever see this before, or know what might be going on?

Running Exchange2003 and Outlook2003.
 
Your attachment table is likely messed up. You have basically 2 options:

1. Run isinteg again and again and again... until there are no more fixes and hope that resolves it. This can be very disruptive in that the store is offline while you are running isinteg.

2. Mount a new store, move all the mailboxes, dismount and delete the old store. User downtime is minimal.

I'd recommend option2; I never trust a repaired database.
