FOP's are really just wiring adapters. They have an input that takes a standard Cat5/6 patch cable from the extension card and splits the pairs into single ports (i.e. 1 pair per port) so that they can be patched individually to an outlet. If you're not using patch panels then they're not useful, you would just wire out your own tail.
e.g. on a 32 port FXS card they are configured with 4 x ports on each RJ45. You would connect this to the FOP and it converts that into single ports.
Obviously if you don't have TDM extensions (i.e. analogue/digital) then they are of no use.
My explanation is really doing no justice as I find it hard to explain but they are straight forward when you look at it. Below is an image that will hopefully explain it better.
