Mitel 6940 Phone Conference

Hi guys we have a Mitel 3300MXE II running software version 8.0 v2. We also have a whole bunch of 6940 ip phones. None of these phone will conference in user with the join button.
So user A picks ups the phone and dial user B. User A hits Add user then dials another number. Once they hit the join button all three are in the call but the original users is not able to hear the calls or had dropped out.
They are also using those fancy bluetooth handsets.

are these fixed or hot desk users ?
if hot desk can they conference from Non 69xx series phones?

By fancy BT headsets are you referring to the new integrated ones?

When conferencing are all participants on 69 series or are trunks or older phones involved.
are any codecs disabled on the system , perhaps its a wideband issue?

They are fixed user but all user have micollad licencing and are created through micollab, so they have access to soft, UCA and desk phones.
The problem i was testing was desk phone to two outside lines. The trunks are ISDN.

Yes the new integrated fancy handsets.

All to outside trunks at the moment. I can try removing wide band,

I assume your users have a CoS which has "Conference Call" set to 'yes'.
