Mitel 5000 USB Failure 1

Ok, this one is weird.

[li]Setting up a new system. On initial bootup, I see is USB Failure - Continue Yes/No (this might have been after Norflash Upgrade)[/li]
[li]Continue and initialize system as usual.[/li]
[li]Initial Load is release 5 - Need to upgrade[/li]
[li]System won't detect USB - No big surprise[/li]
[li]Reboot, reseat, no luck[/li]
[li]Grab Spare Controller chassis - Same Issue[/li]
[li]Swap out CPU Same issue - WTF?[/li]
[li]Have 3rd system with known working parts available[/li]
[li]Install CPU into 3rd system - USB failure again (It asked for Norflash Upgrade surprisingly)[/li]
[li]I guess it must be the Flash Card - right?[/li]
[li]Nope, Brand new flash card, same issue[/li]
[li]Put 3rd system back together with all the original parts - USB FAILURE!!! WTF?!?!?![/li]

WTF Alright!

Maybe try an upgrade via TFTP and then see if it still happens.

Maybe Processor Card is blowing USB on the chassis? <-- Stabbing in the dark.
Are all USB ports not working, both the chassis and Processor Card?

Not that up on the 5000. Are the Flash cards provided by Mitel? Otherwise is it a brand issue with the Flash card? Is there a different brand you could use?

CF Cards are serialized from Mitel. You can't just use any flash card.

The TFTP upgrade seems to have cleared the issue. Nasty stuff.

You know i have seen this before on v5 systems but never got to the bottom of it, i have only started using usb upgrade recently from v6 on wards so i could not comment on the USB port not working.

CF Cards are serialized from Mitel. You can't just use any flash card.

Officially NO technically YES i have cloned CF cards for backup purposes as the turn around time if the CF corrupts is to long for some customers

Yes I have cloned as well. It will work but you can't register with AMC as there is no Hardware ID associated.

Sounds like it was a software/driver issue in the kernel.
I just assumed that it may not have been detected because of a hardware issue stopping the driver from loading.
