Missed calls

One of my customers is complaining the phonemanager reports answered calls as missed calls as soon as you answer the call. Has anyone seen this before ?

PM 2.1.10 and IPO 2.1.24
 
I am seeing the same thing in both Phone Manager, and the Call Status app.

No consistency, some calls appear to be missed, that are not.
 
I thought calls that went to voicemail were considered missed calls. is that what you are seeing?
 
No, I can be sitting at my desk, and pick up a call and it will log as a missed call.

This seems to happen most often when I asnwer a call sent to our Huntgroup, but also happens on calls directly to my extension.

Calls left to voicemail also do log as missed, as they should.
 
Anyone have any useful help here?

Could I have something configured improperly in either the IP Office or the huntgroup?

Why do calls which are answered show as missed or lost, when they are being answered properly?

Is there any setting which will tell the IP Office to not consider a call lost or missed, unless it has been trying for a certain period of time, say 5 secs, or so? All my missed calls(which are not missed) show as the wait time 0 secs.
 
Have you considered upgrading the IPO to either 2.1.27 or R3?
