Microsoft Office Document Imaging

I'm looking to add an annotation to an image in Microsoft Office Document Imaging. I'd like this to be a permanent annotation that is inserted into every page automatically upon opening the document. Kind of like a template with an embedded annotation.

If anyone has any recommendations it would be greatly appreciated.
