Microsoft Excel 2013 - Creating Database Table from Summary Table 1

There's a solution I've used over the years from John Walkenbach's The Spreadsheet Page site.

He gives great instructions for converting a summary table into a database table.

My problem is this. I have not been able to figure out how to execute this solution in the 2013 version of Excel.

I'm gobsmacked by your brilliance! I don't know if I would have ever figured out to use the PivotTable wizard that's not very visible in Excel 2013.
