Microsoft calender control for oraclew form 4.5

Currently working on a oracle database software,having many text fields with date coloum.

I wanted to know how to create date calender from form 4.5

I m using Microsoft 98,with oracle7 and form 4.5.

I have succesfully created with form 6,but unable to create
the same from the form 4.5 version.

Kindly tell me plz in details step by step
