Installed a Sandisk Sansa Clip 2gb mp3 player/radio with a 1/8 stereo jack to rca jack... no audio output at the rca plug, but there is audio output on earbuds.

Any help is greatly appreciated!

Depending on how the 1/8" stereo jack to RCA plug adapter is wired, may be a clue why no audio is present.

If you separate the right and left channel outputs, and connect a testset in monitor mode to each channel separately, do you get any audio signal? No = there may not be enough signal to drive the testset from the player.

As far as the adapter is concerned, do a continuity test to determine if it is wired correctly and not shorting the right or left channels.

Next, there could be a problem with the MOH port on the Merlin Plus control unit, or the feature is not programmed correctly. This feature is administered from the console set. See below for instructions from the Merlin Plus System Manual:
--------------------------------------------
MUSIC-ON-HOLD
1 Enter administration mode.
The red lights go on next to Administer and Restrict.
2 Dial one of the following codes to set the volume you prefer:
> Dial *12 to turn off Music-on-Hold.
> Dial *13 to set the volume to low.
> Dial *14 to set the volume to medium.
> Dial *15 to set the volume to high.
3 Touch Administer.
The console beeps twice.
4 Continue to administer the system, or leave administration mode.
-----------------------------------------------
The MOH port on the Merlin Plus is designed for an 8 ohm impedance source. Since I could not find any specifications for the MP3 player mentioned, I have no clue what this player's capabilities are.

One method to test the MOH port would be to use another signal source like a boombox or a test toner. Then place a call into the system and put the call on HOLD. Check from the calling phone for a tone or music after checking the system programming for settings.

Hope this helps!
