Menu item price levels

Hello,

I am curious to how do we know what are different price levels on a menu item in micros connected to? (price 2 to 10). Like who do we or the POS select these prices? Are these only dependent of serving time periods or are they usually used for different size of items too? (like $5 for small pizza, $5 for medium etc)
