Manual for AD(A)-R unit

Does anybody have the manual for the NEC AD(A)-R unit? It's a recording adapter for Dterm i phones and has a bunch of dip-switches on it and I need to know how to set those switches for it to work.

Even if you don't worry about sending the manual, just describe here what those switches do, that will help.

Off = No Output (Default)
