Making Reports from Forms itself

I Builded a Access Form that users have to input information in feilds.
I want the information they put in to generate a report.
Do I have to put something in the Form to transfer it to the report or do I deem the report to go get the information from the form

I have some Combo boxes that are tied to tables.
I want it to input the selection to the report when clicked on one or multiple selections from the combo boxes.

What im trying to build is a client Progress Note.
Were the information inputed and select will display in a report.
If you get me examples and steps i can take it from there


 
You can possibly get a head start on creating a report by saving your form as a report. There are many methods for filtering a report. Look around and if you don't find something come back with more detailed information.
