Making forms on word - Possible?

Hi - I have a customer who wants to make forms on word so that he can bring them up anytime and fill in the blanks.

But I don't know how to do it without wrecking the formatting everytime I try to fill in the blanks.

Is it possible to lock only certain parts of word so that I can make fill-in-the-blank type forms?
