Making a questionnaire form in excel 2

Does anyone know of a good template for a questionnaire form design for MS Excel? I need to generate a form for a questionnaire in excel, and to make the document look as little like excel as possible, and for it to be very easy to use (i.e. dropdowns, tick boxes, etc) and also for the results to be easily transfered into another hidden worksheet for processing my end. The survey will be sent out via email, and then sent back for the results to process.

Please note, i have not done this before so some guidence would go down very well Thank you for your help already.

your page format is up to you...on the look and feel stuff,
but if you want to force the user to select from lists then

highlite the cells that the user will fill in...

then select data validation, located under the data menu, then select "list" from the criteria/allow: drop down

you can either ref cells containing valid values or type a list seperated by commas.

when you click on any of the origionaly highlited cells, a drop down will appear, if you try to type in those cells an error msg will appear, and block the input
 
what i have done, is made up three sheets. One is the display sheet, the other the data sheet, and the final is the results sheet. I think this way will work, but just need some guidance as to finishing.

The fist sheet, i have put what i want and how i want it, sfor the view of the user. So, it has the logos, questions, drop downs, tick boxes, option boxes, etc. I have got the data for the dropdowns from the data sheet. So i have say, question one equals dropdown Yes, No, Dont Know. These answers were retrieved from the data sheet. And look good in the dropdown. Then i use the 3rd sheet as the results page, so the choice that they make goes into this sheet.

This all works fine. But, i have a couple of issues.

A: how can i get a text field in there. it seams like it has been greyed out so i cannot use it. I want them to endter some free text, and for this to also go into the resutls page, but i cannot see how i can do this? Help?

B: i am sending this to a number of people, and i want an easy way to get all this data to come back and then collate. How can i do this when i have lots of seperate docs coming in. How do i add up all the resutls from the worksheets?

Ideas? and lots of help please? Thank you for your help already.

Regarding your need to merge the data from the many questionnaires, you could potentially link these files, but my recommendation would be to use VBA code to merge the data from each of the questionnaires.

This would involve code which opens each questionnaire file, then copies and pastes the data into a "database sheet" within your "summary" file, then close the questionnaire. The data would be added to the bottom of the preceding data from each questionnaire - i.e. one row should contain the data from each questionnaire. The data will then be in a "data format" which can in turn be properly analyzed - with database formulas, etc.

The routine would "loop" to merge ALL the questionnaire files in ONE step. This of course would require that you rename each questionnaire file with a sequential number when you receive it back (if you haven't already done so) - example: "questionnaire_001",
"questionnaire_002", etc.

In order to help you out any further, it would be made a whole lot easier if you could offer a copy of your questionnaire file, with the questions modified if necessary so as not to disclose the nature of your questionnaire.

Dave - if you do not mind, i will email the questionnaire to you, in order for you to recommend a good plan of action. i think that the linking up inot a database would be feasible, but i lack the sort of vba coding expertise to get this done. i have developed database before, and am learning quite rapidly, but through more design views rather than the vba.

I could spend the next 2 weeks trying to explain it to you using this medium, so, if your provide me in detaildetail what you need and give me a time frame I can have it done at no charge. It's great to see that Tony has also "taken up the challenge".

You should ALSO send the actual questionnaire file to Tony, as it will be necessary to have the "complete picture" in order to develop a proper solution.

One other thing, with regards to presentaion. If i am going to add this as an attachment to an email, do you, or anyone else know of a way in which i can change the icon, so that it does not look like a standard excel document? Thank you for your help already.
