major issues. phones lose connections

I have a customer that needed some new licensing and up upgrade. He went from 8.1 to 10.1.0.7
digital adp 30 B2 all were upgraded and show the correct level of 10.1.0.7 We replaced all of the telephones from 5410's to 9508's. There were few hiccups, but then each phone went through the process and downloaded the firmware (.60). they Worked great last yesterday. this morning the phones were locked up and the buttons wouldnt work. i rebooted the system and they all started working. Tonight They all show on the System status, but the phones are not working at all. Many of them say waiting, Then they might come up with the details on the buttons, and the display, but the buttons dont work. Unplugging them and plugging them back in sometimes help... for a little bit.. sometimes making a call brings the buttons back to life, sometimes you call another extension and it rings... and continues to ring, no matter if you try to hang up on either end. THe only way to get it to stop is to unplug the calling phone . The extension lights on the digital 30 are flashing green all over the place. even though there is no one but me in the building and the phones are certainly not being used. THis is an office that is just trying to open up after the shutdown and this is a disaster. I Cant figure out an answer. I need help. They need help,
 
For the digital expansion module can you see it under status? if its not there those handsets wont work that are connected to it.

Or is the expansion on a different level of firmware. they should upgrade with the system, but have seen when they dont. under status it would greyed out if different.
 
the control unit and the expansion modules are on the same revision level, The modules do show up on system status, but the individual extension do not show the full range of buttons.
 
i guess thtat is just the call appearance buttons
 
update and further details

Ip 500V2 R10.1.0.7
slot 1 empty had been an analog 16
slot 2 Digital 30 B2 R10.1.0.7
slot 3 removed an old digital 30 empty now
slot 4 Digital 16 B2 R10.1.0.7 This started in slot 3 but was moved to try to change the problem

All units are up and working as intended..... until you reboot the system. When you reboot the 2 diital station units start having groups of lights blink and the phones dont come up properly, buttons dont work, display is missing info, etc. that is with a normal system reboot.

to get it to work properly.. i power down the expansion modules, then the control unit . When everything is down, i put power back on the outside unit. the ds 16 b2 in slot 4 and let it power up completely. Then I power up the DS 30 b2 in slot 2 and let it
power up as far as it can. Then i plug the control unit in and everthing comes up properly and all extensions are working properly

I have recreated this 3 straight times. this shouldnt have to be done with a normal reboot. Anyone seen this and have a solution??
 
I would treat it as if there was a problem with one of the Modules.
These ones been in service awhile
Turn off
replace patch cords. turn on, Test...
if good your done.
If bad turn off, disconnect module, turn on test.
Replace power supply....


 
What I do know is that blinking ports typically means the phones are trying to upgrade. As a personal opinion if I understand right, you have module ports in the middle of things that aren't being used. I wouldn't say that is the problem, but unless you have a bad module port, I wouldn't do that. You should also look at the control unit section of the config and see if it is still programmed for the missing modules. Backup the config and remove them from the config and reboot. That will remove them from the config. If it causes problems when you do that just reload the backup config and that will at least get you back to square one.
