Looks nasty. Reinstall/restore from backup if you have no Avaya support, else call them if you have maintenance.
Its an LDAP problem, it requires a level of access only Avaya has to correct - if you have an account at support.avaya.com, this would tell you about it:

The customer has support with Avaya. but as we are installing they are saying we need to pay to sort it out as it is configuration issue.
We will wait a couple of days and get back to them.
