Mailbox Password disabled

Has anyone ever seen an issue where a mailbox password gets disabled even though the user swears they entered the password correctly? I know its probably user error but I had to ask.

Yes, usally they are HACKERS

OLD ROLMEN WORKING ON NORTELS AND AVAYA

It may be someone else trying to get in to the mailbox in error or maliciously. Someone may think they can get to their mailbox from any phone without having to enter the mailbox they want, or someone has an "alternate agenda".
