Mail Merge from Access

I have encounter 2 problems while doing the mail merge.
1. when the doc opens from Access form, it only
contains ONE record from my user's machine(should be
more records showing on the different pages).

2. If the date filed is blank in the source table, the doc
will show 12:00:00 AM

Hope you can help me.
