Mail Merge Customization

Suppose i have in Access a query with 10 records. Out of these 5 to one location and 5 to another. i.e. same address but different details.
I want the address once and the details like Chq no, DD No etc in a table. these details should appear in seprate rows in a table.

Can any one please advice. It is very urgent.
