Mail Merge and Grouping

I was wondering if by any chance anyone has been been able to create a mail merge in microsoft word with 2 groupings. Example Data:

Country City Name Title
USA Dallas John Manager
USA Dallas Brian Assistant
USA Florida Tom Assistant
Europe London Liz VP
Europe France Carmen Director
Europe France Bill Assistant

The result should be:

USA
Dallas
John, Manager
Brian, Assistant
Florida
Tom, Assistant

EUROPE
London
Liz, VP
France
Carmen, Director
Bill, Assistant

Here is the code I am using:

Correct, I am trying to create a word template in order to generate a directory based on a public contact folder from Microsoft Outlook. Currently our receptionist updates the employee contact folder in outlook and then she has to update the contact list template in word. This way she only updates the information in one place. She can mail merge from outlook and select the contact list template click merge and be done.
 


This REPORT is the kind of thing that is extremely simple to fo in Excel using a Pivot Table. I Copies the data into Excel, parsed it, and did the Pivot Table report ALL in less than 2 minutes.

It has to be done in word in order to use the template with our logo and formatting for our board books. Excel wont' work.
