Mail form.

I'm a bit confused on how to create a mail form that sends feedback to my e-mail address.
Here is what I have:
A name field.
A message field.
A "clear" button.
A "send" button that sends both fields to my e-mail.
I need an action script for the "send" button.
Any help would be much appreciated.
