I have a Magix r3. It has a DS100 card. In the error log it shows many error, DS1 Misframe, DS1 D channel fail, DS1 slip, DS1 Loss of signal. The carrier tells me that channels 18-23 are locking up and causing the problem. They say it is the PBX. Has anyone ever heard of a DS100 card being defective like that?

It could happen sure.
You could loop the jack to perform a loop back test.
If you had a power outage AND the 100D is in a secondary cabinet then perhaps you could power cycle the system in the correct order.

Parts do fail.
