One of my users phone is stuck in such a way as all incoming calls to his extension are sent to the console, he cannot make any outgoing calls, and the only softkey enabled is "Forward". He claims that he didn't do anything to the unit, so I am a little lost on how to reset his line. By the way, I'm a sort of last choice phone guy, my real job is Unix SA. The regular guy is on vacation.

Thanks in advance..

I'll assume the person who reported the problem has a digital phone. Press the "Forward" key on his phone and check if "CALL FORWARD CANCELLED" displays. If the phone doesn't have a display, check that the arrow, on the LCD next to the "Forward" key, goes off. Next, place a call to his extension and see if his phone rings or if it still goes to the attendant console.

As far as not being able to place outgoing calls is concerned, log into the PBX and go into LD 11 (you may first want to make sure that this individual is supposed to be making off-switch calls). Then print the TN for an extension that you know can make outbond calls and compare it to the TN for the extension that can not.

>LD 11
<Some Text will display>
REQ: PRT
TYPE: DNB
CUST 0
DN <the phone's extension>

*** Press <Enter> at the next 3 prompts and at NACT ***

The result will give you the TN for the particular extension. Next...

REQ: PRT
TYPE: TNB
TN <the TN obtained previously>

*** Press <Enter> at the next 3 prompts and at NACT***

You will want to compare the setting for NCOS on the extension that can dial outbound vs the one that can not. If the NCOS settings are different, you can modify it on the phone that can not dial out...

REQ: CHG
TYPE: <the model of the phone ie: 2616>
TN <the TN for the extension previously obtained>
ECHG YES
ITEM NCOS <number> (the same as the NCOS for the phone that can dial out)

ITEM <Enter>

Then try placing an outbound call from the phone in question. Note that there are several other things to check if none of the above works but these steps are the first and easiest places to start.

Also check out the TAC alert in this forum for release 25.40 and 3900 phones, involving forwarding activation/deactivation, and loss of call registers.

Hey probably this is not the case but I found this alert from Reusser that could be helpful:
Date: May 30, 2002
Number: 2002-092
Call Registers Lost after Call Forward Activation / Deactivation on X11
Release 25.40 and X21 Release 1.1 with M3900 Phase 3
This Product Advisory is being issued to alert you to an issue with X11 Release 25.40/X21 Release 1.1
software and M3900 Phase 3 sets that will degrade system performance and eventually restrict calls from
being made.
Problem Description:
For sites running with X11 Release 25.40 or X21 Release 1.1, every time Call Forward is activated and
deactivated on an M3900 Phase 3 set, a call register is lost. If the patch identified below for this issue is
not installed, system performance will be gradually degraded, resulting in increasing delays in obtaining
dial tone as call registers are depleted. Eventually the Meridian 1/Succession CSE 1000 may completely
run out of call registers resulting in the all telephones on the system being unable to make or receive
calls.
How quickly this issue will occur depends on the number of call registers in the system, how many M3900
Phase 3 sets are in use on the system, how frequently Call Forward is used, and the length of time since
the last the occurrence of a warmstart/INI. A warmstart/INI will recover the call registers. To date, only
one site has reported the symptoms of slow system performance.
Expected Scope:
The problem may be observed on any processor type running Meridian 1 X11 Release 25.40 or X21
Release 1.1 with any model of M3900 phase 3 sets. Succession CSE 1000 Release 1.1 supports M3900
sets through Remote Office only.
The problem will not be observed with M3900 Phase 3 sets running on any pre-X11 Release 25.40 or
pre-X21 Release 1.1 software releases.
The problem applies to Phase 3 M3900 sets only.
Solution:
MPLR16508 is now available on the Meridian 1 Patch Library to address this problem and must be
loaded on all X11 Release 25.40 and X21 Release 1.1 systems using M3900 Phase 3 sets in order to
prevent the problem from occurring. A warmstart/INI is not required to activate the patch, but it is highly
recommended that an INI be performed as soon as possible to recover any Call Registers previously
affected by the problem.
The patch is planned to be included as quickly as possible in an up-issue of the Release 25.40 MDCS, as
well as in an up-issue of the Release 25.40 Dependency List.
