I have a problem whereby the Lync enviroment has multiple front end servers so when the LBG server communicates with the front end pool, the messages dont go back to the correct front end server because LBG is incapable of caching DNS entries. Apparently, the way around this is to use a hardware load balancer but would like to get other peoples reviews on this. The link below describes the problem with a SIP messaging flow process :
