What is the message trying to tell us?

As the message says, you have activated a volume group recently without including all disks on that vg!

so the quorum got effected!

What was the recently varied on vg?

look into the disks on that vg and see which one is missing!?

MAJOR/MINOR DEVICE NUMBER
002E 0000

I am not sure how to check it with this major/minor numbers.

look in ls -l /dev for the device with major device number 46 (hex 2E=2*16+14) and minor device number 0

Look for the numbers 46, 0 in the "size" column. The device will be the name of the VG.

This is weird...
I am seeing this:

$ ls -al |grep vg46
crw------- 1 root system 46, 0 Dec 04 16:20 __vg46
 
Well, look for missing PVs in the output from this command

lsvg -o|lsvg -ip
