LUDN listing 4 digit DID & some 7 digit NXX-XXXX

Hi,
I have a site where the NXX is 775. There are a few ranges of DID that we buy from the telco in the 4000 and 7000.
Curiously enough, I noticed that in the LUDN (snippet below) there are some listings of the NXX-DID giving a full 7 digit.

Can someone explain why there would be 7 digit listings there? How could a ranger have been created to make the LUDN listing show the NXX (none of my other sites have this)?
Also, the listing 7754000 is in use, so there is some error here.

All it takes is for someone to program the full 7 digits on a phone instead of 4 digits and you will see this. Perhaps if you printed a DNB of 7754001, it might lead you in the right direction.
