In short, there is an error with the card in slot 4, the action taken should be run the 'show tech-support' command and capture it's contents and contact Extreme TAC. Is this error just a single message or are you getting bombared with them?
I'd open a ticket with Extreme TAC, or whoever your service provider is for the equipment. You could run an extended diagnostics, but keep in mind it could put the card into a 'fail' state in which case it becomes unusable. What Extrmeware on you running?
