LSI MegaRAID SAS 9260-4i beeping, Server will not boot into OS now

LSI MegaRAID SAS 9260-4i is beeping.
The WebBIOS utility showed 1 failed drive.
I replaced the drive.
The Windows Server 2012 R2 OS is installed on an SSD connected to a SATA controller on the Supermicro H8SGL-F-O motherboard.
The server will not boot into the OS now.
When I boot the server the RAID card beeps and I can access the WebBIOS utility but that's it.
I would expect that I should be able to boot into the OS and that RAID data volume would just be degraded until the new drive is rebuilt.
Why isn't it performing it this way?
I've attached a screenshot of the WebBIOS view.
Any help is appreciated.

Slot 12 warning a physical disk is not being reported, either it is bad or issue with slot, most like a bad drive. The PD Missing error may be interfering with the booting (unlikely)or the bios reset, and the raid 6 is set to be booted from, which has no OS to boot from. Technically the PD Error is a "warning error", not a fatal error, and should not get in the way of booting. Check the boot sequence.
