Lost Three DS Mods at once 2

Hi, we have a site with a 500V2 R9 with
Combo ATM4 in slot 1
Combo ATM4 in slot 2
DS8 in slot 3

All phones are dead, flashing on and off randomly doing the Christmas tree colors on the buttons (1400 series).

Had the IT people who were there when it happened, do a hard power cycle for 60 seconds and plug back in. No luck.

I did a remote reboot, no luck.

Status shows the modules, but with no extensions plugged in. On the mods, 9-12 are lit for lines present and the VM answers, but all DS ports seem to be down.

Could we have lost 3 mods at once? No power issues, failures, etc that I know of. The IT peeps were there when it happened.

Could it be 500V2 itself? I still get the amber heatbeat lights like always, just no lit DS ports on all 3 mods.

We see this all the time. I bet if you listen closely the system is clicking. Normally its one of the combos that got zapped and once you pull it system will come back up.

I would label all the cables and disconnect them and reboot. I have seen an individual module flip out if a PC gets plugged into it. I don't know what it would do if a PC got plugged into a base card.

That sure makes me believe that someone plugged a computer into the IPO and something went bad. Unplugging everything from each card and module (label if not already), and then trying one card at a time and seeing what it will boot with. Likely got a bad port somewhere.

The truth is just an excuse for lack of imagination.

I would start with the base cards. Leave the cards in but unplug every station cable and reboot. Also label and unplug the modules from the back.

Just talking to a coworker. Recently he has 2 sites that a bad combo card brought down the entire digital bus. So popping the cards out probably would be a good idea.

If the combo cards have been zapped it is probably from a lightning strike, in which case the only sensible course of action is to declaire the wholw system suspect as you do not know what components may have been weakened 7 it is only a matter of time before they fail
the customer should claim for this on their insurance
