Looking for a tool to construct questionnaires and tests

Hello everyone,

I am looking for a tool for Lotus 5 with which someone who does not know how to use Domino Designer can produce questionnaires or tests, deciding whether a question is a yes/no or multiple choice with one or with more than one answer or a text question. Depending on whether the person constructs a test or a questionnaire he must be able to choose how the results are dealt with. With questionnaires the results sometimes have to be very specifically sorted.

Any ideas? I would be very happy for any tips.
