The 523 Modular plug connector, the only company that may still be manufacturing this item is CSI/Suttle Apparatus, 800-852-8662. This would be from their OEM - Custom Products section. Their part number would be SE-523A4 or SE-523A6, 4 conductor or 6, respectively.
The last time I ordered these was in 2002.

well in looking around, I ordered some Allentel, AT523A-4's which appear to be correct thing, otherwise I will keep shuttle in mind

I got the parts from graybar today and it looks good,
I tried fivestarphones.com and they could only do 25
