Long distance issue

I am having an issue with a school district dialing long distance. They are centered around an option 81 with primarily option 11's at 90 % of the schools and the other 10% are using cisco call managers. The issue first arose on the cisco sites on fax machines not being able to call LD. All calls at all locations require an authorization code( so 9, 1-XXX-XXX-XXXX, then woman's voice asking to please enter authorization code, enter 7 digit auth code, then call goes out). The fax's will work 6 out of 10 tries calling LD and the other 4 will say you have entered Wrong auth. Code. I tried same test from nortel sites and here same woman's voice and calls always work. So I at least assume request for auth. Codes is coming from same source.(I figure LD provider). What could cause this instability?( my novice thought was maybe LD pri was full, and fail over was pushing call to another pri that access code was different). The weird thing is no one with cisco ip set is reporting issue with LD, only fax's. Within the same week, I now have had a total of four nortel phones from different campuses are reporting issue with auth. Code that state LD has worked before. On these sets as soon as you u are prompted by same woman to enter auth code and press the first digit, you just here tone that won't go away. On one particular phone which is a main secretary with multiple lines(she cant dial LD from anyline)She has a coworker with a DN of 0030. 0030 can always dial LD, but secretary can't dial LD when using 0030 from her phone. I can't imagine what could have happened b/c I am only one doing any kind of work on system. These problems may not be related, it's just weird that within same week all the sudden folks are reporting issue with LD.........sorry for the novel, any tips or advice would be greatly appreciated!
 
First, where are you located? Here in Washington we have long distance service via the State of Washington. They have a service (SLD) with a LEC that is using SIP trunks. We began experiencing this issue and they had to create a Fax friendly route.
 
I would ask the question : Are you using anything apart from G711 on the links between the Cisco and Nortels (assuming that they are IP). If compression is happening, it may be possible that tones are not recognisable.
