Long Distance ISDN PRI down

I have an ISDN PRI that's down, The carrier says the problem is on my end. We have a CS1000E, there are two other ISDN PRI circuits installed in the same module that are not having any issues; We are using TMDI cards for all our T-1's. I have down loop backs from the smartjack back my switch and it will clear the red alarm on the span but of course not the DCH alarm. Also, I swapped known good cards and cables but no change. Below is the printout of my route, d-channel, and trunk member. The far end switch is set-up for Nortel, at least that's what the technician told me. I have both Route and DCH IFC set for D250. Mind you that we have not had any issue in the past with our long distance and this just happen out of nowhere. Any help would be greatly appreciated.

Sorry for my haste in typing. It should read, I have done loop backs from the smartjack back to my switch...

 
The problem is most likely not programming if it has been working and just failed. I assume you were having no other issues when it was working??
Do you have a plug you can use to give an External loop back to the telco and have them test? When the telco does loop tests, it does not test all of the smart jack on the wall, giving them an external loop back will allow them to test the whole card, as well as your patch cables, if you unplug from the back of the PBX.
 
I do have a loopback plug and I connected it to the port on the smartjack that the pbx is connected to. The carrier send the loopback with no issues.
 
Also, When I unplug the cable from the smartjack and plug it right back in, the dch will establish and when I try to make a call I get a PRI222 error. Shortly thereafter the DCH will release. See Below

When I put a loopback towards the CS1000e the red alarm goes out and I receive the following messages;

When you put the loop back on to the carrier, they do not need to put a loopback on it themselves, just run a pattern through your loop back.
 
Exactly, with the plug in the smartjack the carrier loop thru with no issues.
