Long distance codes- Through CCA

How can we setup a long distance code through the CCA?
This must be a 4-5 digit code that callers must input before beings able to dial +1 and +011 calls.

Any ideas?

CCA - Copper Clad Aluminum - A junk type of CAT5 or CAT6 cable that won't pass certification; but not sure how it relates to long distance codes.

Just to show that one person's shorthand can be meaningless without sufficient information.
