Logoff doesnt work on OWA e2000 SP3

I have exchange 2000 installed, service pack3 plus the post SP3 roll up patch and windows 2000 SP4

outlook web access is working fine, ive added a simple html refresh page to send

http://mail.domain.com

to

http://mail.domain.com/exchange

The user is then prompted to login and can do succesfully

The problem is when they try to log off

The log off icon link points to

http://mail.domain.com/exchange/user/?Cmd=logoff

Upon clicking log off I get the following error in IE:

"Server Application Error
The server has encountered an error while loading an application during the processing of your request. Please refer to the event log for more detail information. Please contact the server administrator for assistance."

The page it redirected to, to give this error is :

http://mail.domain.com/exchweb/bin/USA/logoff.asp

The logoff files does exist in:

C:\Program Files\Exchsrvr\exchweb\bin\usa

Can anyone please suggest a fix for this?
