Login Form

Heres an easy one!
I am trying to create an asp.net login entry page ,i wish to begin with a simple page connecting to a database.

Eventually i will need something which uses windows authentication.
I am using asp.net VB.net and sql server.

Can someone point me in the right direction to a tutorial or some code that can help?
