LJ 5000 13.5 paper jam open top cover

My Laserjet 5000 jams every single page. The printer screen reads, "13.5 Paper Jam Open Open Top Cover"

I open and close the top cover and it finishes printing the page. Does anyone know this problem or error code and how to get it printing again?

The printer has been showing "Perform Printer Maintenance" message, too, but I think the two are unrelated.

Thanks for the response. Just thought I would share my resolution:

The fuser has tears in it, causing the paper jam and I had to have a maintenance kit installed.
