List Parameter Value 1

Hello,
I have a database that is run fromt he switchboard. When you click on the new report, it prompts you to enter a date. The are twelve date possibilities. The users would like to have a list of the tweleve dates, and just be able to choose one of the twelve dates as a parameter to run the field. I have never attempted this, and use some guidance.

Create a form with a listbox. Add the possible dates to the listbox. In the Click event for the listbox, run the report based on the value selected.
