I have to install a line side T1 card in a second shelf of option 11. It is is slot 5, so the tn is 0-1-5. What is the addressing for line side card for that shelf? I am assuming it isn't set (off, on, on, on ). How is it counted on second shelf, would it be card 14? Some clarity is greatly appreciated.

I don't think those dip switches are for the PBX card slot, it shouldn't matter. You program it just like an analog card.

I believe it's an address and doesn't really matter what slot you put it in.
I think I remember it can be addresses from 0-15.

The address is used to link the T1 Line interface together so all you need is one link to talk to them, if you place the card in cabinet 2 it takes on 2 slot numbers.
your first 16 channels are on the first slot and the second 8 channels are the next slot
