Line not disconnecting

Just curious but what would cause a line not to disconnect.
One of my user finished his phone call and hung up but it would not hang up. The light still flash saying the he is still on the phone. I tried releasing and all the stuff that I can thing of to disconnect the call but it would not so the only other option I had was to disconnect the cord.
Any idea as to why it would do that. Maybe the telephone profile is corrupted or something that is why it would not disconnect the phone.

Thanks for any information that I can get.
 
Is this an isolated case? if this has happened before - take note the next few times of the display. What is the route/trunk info. does this always happen on the same trunk?

You see were i'm going here. If this is an isolated case - you may have fixed by disconnecting phone (disabling and enabling would also do the same). If this re-accures, we need to isolate the situation - same phone, same trunk group etc.
 
You didn't state whether this is on external or internal calls, or both. If external calls, check your route settings & make sure FEDC-NEDC are both set to ETH.
 
This was with an external call. Yes, this was the first time that happended on this phone. The last time this happended also was a while back on another telephone set.
But I will keep a tab on it and see if it happends again on the same phone.
