I have a user whose mailbox I have configured for 10 meg attachments by going into Delivery Restrictions then Receiving Message Size and putting in maximum KB of 10240, which should be 10 meg.

yet when I try sending this user 5 meg or larger attachments from gmail or hotmail it gets denied, getting
Action: failedStatus: 5.2.3
error.

I can however send an email from my company address to hers and the 5 meg file goes thru.

what else do I need to look at?

There are 4 places to check message size limits, global settings, the smtp connector, the smtp virtual server, and the mailbox. You need to check all of them. If you recently changed a mailbox limit, the change can take up to 2 hours (plus AD replication latency) to take effect. This is the default time that mailbox information is cached by DSaccess. If you want the changes to take effect immediately, then you can restart the IS.
