Lightning/Surge protection at KSU?

I had a MICS 6.1 go down just recently due to what I suspect was a lightening strike. (Storms passed through the night before). The system is configured with one LS/DS card and DTI card configured for PRI. On my system I had 1000v battery backup and double redundacy with surge protection. I also had a Towermax Surge protection on the CO lines. The system was frozen in startup mode, and would not give any display on the phones. The T1 connection from the carriers smartjack was the only area not protected inside my design. My question being what could of been the possible cause of my system failure and what should I do to prevent this from happening again. Oh yeah, I had to replace the NVRAM cartridge and the software to get the system back-up! This facility is a medical one so I need to ensure that this will not happen again!
 
Power spikes will go through UPS units. On one of my most sensitive customers, I've put isolation transformers. On the T1 circuit side, fast tripping gas protectors should be all you need and those should be there from the Telco.
