Last two digits of outbound calls sporadically flip-flopped

IP500, analog lines.

Periodically the last two digits of a dialed number will get flip-flopped somehow.

Eg: User enters 123-456-7890 but it's terminated as 123-456-7809.

Odd thing is the call logs show the correct number is dialed, yet it's terminated incorrectly. I'm guessing this may be a carrier issue and not related to IPO but I want to cover all the bases. Thanks.
 
Just run monitor and tell the clients to write down the times that they have the issue. It will give you some proof but the carrier may (will) probably still say its your fault.

 
I have done that and the monitor says everything is going out correctly. It will be interesting to see what the carrier has to say.
