KX-T7601X (TDA USB module) reliability ?

I have a site where the 7601 USB module is consistently failing. The set has been changed, the PC was swapped with another, and the AC power and ground to both the PC and KSU was checked out and found to be clean and properly wired. (Both units are on powered by the same breaker panel and are both on the same side of the neutral.)

The modules last anywhere from a week to two months before failing.

Anyone else have a reliability problem with these modules?
