Known 2.1 (249601) Problems?!

I'm sure these have been posted but we are having the current problems.

Our Lantana guys put this patch to rid the problem of hold music coming on before VM at our IDF or location B. Since then the problem has come back.

Problems:
1. Hold music before VM ONLY at our B location.
2. We cant receive VM from internal users, only outside people.
3. Random dropping of phone calls when trying to dial out.
4. Forward on no answer does not work.
