Kernel patches

I want to install some kernel patches but I can't seem to get any downtime. How bad is it for the system if I install kernel patches while the system is up and running? Also I know that I am supposed to immediately reboot after the install but what if I wait a day or two? I've done it this way in the past and was told that it is incorrect but I didn't encounter any problems. Do the new patch files just not get used until the reboot or what?
 
I think it depends on what you are installing. I've encountered problems when updating certain share libraries, like libthread.so, for example. Updating this library without a reboot can cause strange errors.
 
I recently installed a kernel patch on a running system and corrupted the kernel, ouch.
