Java LDAP Browser Logon

I am using a Java LDAP browser ver. 2.8.2 developed by Jarek Gawor. If anyone has used this in the past I am having problems making a connection using user info. Anyone know what the proper way of enter "User DN" to connect? I know I have the right host and port as I can Fetch DN's and logon using an Anonymous bind, but when I put a username and password the connction fails.

Thanks for any help.
